Who we are

Fairhaven Claims is a trade name of Windward Leads LLC, a Delaware limited liability company. We operate this website. We are a private company and are not affiliated with the Social Security Administration or any government agency.

What we collect

  • Information you give us. Name, phone number, email address, state of residence, and the answers you provide in our eligibility questionnaire, including information about your employment status, age range, medical treatment, and the status of any disability claim.
  • Information collected automatically. IP address, browser and device type, pages viewed, referring URL, and advertising identifiers such as UTM parameters, Google Click ID, and Meta Click ID.
  • Consent records. When you submit our form we record the exact consent language displayed to you, the time of submission, and a session certificate generated by ActiveProspect's TrustedForm service, which captures how the form was completed.

Health information

The questionnaire asks about your ability to work and whether you are under a doctor's care. We are not a healthcare provider and are not a covered entity under HIPAA, so HIPAA does not govern this information. We treat it as sensitive and limit its use to matching you with a representative.

How we use and share it

We use your information to evaluate whether you may be a candidate for Social Security disability benefits and to connect you with one or more independent attorneys or accredited non-attorney representatives. We share the information you submit with those representatives. We also share data with service providers that host this site, validate phone numbers, place calls, and store consent records.

We do not sell your information to unaffiliated third parties for their own marketing.

Calls and texts

If you submit our form, you consent to receive calls and text messages at the number you provide, including via automated technology and prerecorded voice, about Social Security disability benefits. Consent is not a condition of receiving any service. You may withdraw consent at any time by replying STOP to a text, telling the caller to stop calling, or emailing privacy@fairhavenclaims.com. Cookies and advertising

We use analytics and advertising cookies, including Google Analytics and the Meta pixel, to measure which ads and pages produce inquiries. You can block cookies in your browser and opt out of interest-based advertising through the Digital Advertising Alliance and Network Advertising Initiative opt-out pages.

Your choices

You may request a copy of the information we hold about you, ask us to correct it, or ask us to delete it, by emailing privacy@fairhavenclaims.com. Residents of some states have additional rights; we honor those requests regardless of where you live.

Children

This site is not directed to anyone under 18 and we do not knowingly collect information from children.
